Adrian Monk (Tony Shalhoub) já foi um bem sucedido detetive da polícia de São Francisco, e se tornou uma lenda por seus métodos incomuns para solucionar os mais complicados casos do departamento.
Mas após a trágica (e não solucionada) morte de sua mulher, Monk ficou devastado e se tornou um obsessivo compulsivo. Sua desordem psicológica o levou a desenvolver um medo anormal de praticamente tudo: germes, altura, multidão... até mesmo leite. Sua condição acabou lhe custando o seu trabalho, e continua sendo um desafio em sua vida diária.
